In connection with the requirements under the Act of 10 May 2018 on the protection of personal data (Journal of Laws of 2019, item 1781, as amended) and the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (OJ EU.L No. 119, p. 1 hereinafter referred to as: GDPR), which comes into force on 25 May 2018. We inform you that:
a) The Controller of the personal data collected is: The Culture and Art Center of Wrocław (Ośrodek Kultury i Sztuki - OKiS) in Wrocław, based at ul. b) The purpose of data collection is to accept applications and register participants for the Summer Guitar Festival 2024. The data is processed for the purpose of organising the event on the basis of Article 6(1)(a) and (c) of the GDPR.
c) The Participant of the Summer Guitar Festival 2024 has the right of access to the content of the data and the right to rectification, erasure or restriction of processing, as well as the right to object, request the cessation of processing and data portability, the right to withdraw consent at any time and the right to lodge a complaint with a supervisory authority (i.e.: President of the Personal Data Protection Office, ul. Stawki 2, 00-193 Warsaw). d) Provision of data is voluntary, but necessary to apply for participation in the Summer Guitar Festival 2024.
e) If data is not provided, it will not be possible to participate in the Summer Guitar Festival 2024.
f) The data provided by the Participant will not be shared with third parties. Only institutions authorised by law will be recipients of the data.
g) The data provided by you will not be subject to profiling.
h) The Data Controller has no intention to transfer personal data to a third country or international organisation.
i) Personal data will be stored for the time required for its use, i.e. until the Summer Guitar Festival 2024 is completed and accounted for.
j) In view of the above, as a Participant of the Summer Guitar Festival 2024 or on behalf of a person under my care who is a Participant of the Summer Guitar Festival 2024, I consent to the processing of personal data in accordance with the aforementioned purpose and for the period set out in clause 8.
k) These terms and conditions constitute a template agreement within the meaning of Article 384 of the Civil Code concluded with the Organiser concerning participation in the Summer Guitar Festival 2024.
